Failure Reporting and Corrective Action (FRACA)

Project Brief

  • KDC provided specialist expertise to our customer for the management of Incident Reporting (IR) for Qualification and Through Life Support.
  • This included the creation, root cause analysis and corrective actions of the Failure Reporting and Corrective Action System (FRACAS) artefacts.

Project Approach

  • KDC assumed responsibility for running the failure review board; assigning actions and assessment of FRACA documentation proposed for closure.

Project Solution

  • KDC provided authoring and management of containment actions, including assessment of IRs to ensure that they were linked to a genuine qualification, test or customer raised failure rather than an anomaly and then raise and associated FRACAS artefact.

Project Outcome

  • Successful management of outstanding FRACAS artefacts, leading to eventual completion and closure.
  • Established a robust system for the identification, management and resolution of future Incident Reporting.
