Project Brief
- KDC provided specialist expertise to our customer for the management of Incident Reporting (IR) for Qualification and Through Life Support.
- This included the creation, root cause analysis and corrective actions of the Failure Reporting and Corrective Action System (FRACAS) artefacts.
Project Approach
- KDC assumed responsibility for running the failure review board; assigning actions and assessment of FRACA documentation proposed for closure.
Project Solution
- KDC provided authoring and management of containment actions, including assessment of IRs to ensure that they were linked to a genuine qualification, test or customer raised failure rather than an anomaly and then raise and associated FRACAS artefact.
Project Outcome
- Successful management of outstanding FRACAS artefacts, leading to eventual completion and closure.
- Established a robust system for the identification, management and resolution of future Incident Reporting.
